Are you preparing to become an Accredited Case Manager (ACM™)? This two-day webinar workshop is designed to provide the practicing Health Delivery System Case Manager with information to prepare for ACM™ certification exam.
The scope of services and standards of practice provide social worker and nurse case managers with a guide by which they can engage in the practice of case management. Understanding the parameters within which the case manager practices assures a uniformity of services delivered to patients in need and assists the case manager in performing the duties required within health care delivery systems. This workshop is designed to provide the practicing case manager with a review of case management practice standards in accordance with American Case Management Association guidelines and ACM certification. It will provide intensives for focused review in domains of case management practice including screening and assessment, planning and care coordination/intervention/transitions.

As required by applicable boards, a copy of this certificate and evaluation form will be retained by ACMA for a minimum of 7 years after the course concludes.
Pat Kramer, Ed.S, ACM, CCM, CSW, NCC is the Senior Director of Case Management for Duke University Health System. She has 30 years of experience in the evolution of hospital case management, working in acute care hospitals in Kentucky, Virginia and North Carolina. Pat holds a Specialist in Education (Ed.S.) Degree in Mental Health Counseling and a Bachelor’s Degree in Social Work (BSW) and is an Accredited Case Manager (ACM), Certified Case Manager (CCM), a Certified Social Worker and a National Certified Counselor.
Pat is a past member of the National Board of the American Case Management Association. She remains involved on a National level as a member of the Public Policy Committee, the Compass Advisory Board and as an ACMA-approved trainer for the ACM Prep Course. She is also a Past-President of the ACMA North Carolina Chapter where she served on the Board for 6 years. These connections allow her to interact with case management professionals across the country on a routine basis.
Pat has presented both across the State of North Carolina as well as nationally to Case Management audiences about a wide variety of topics from care of the psychiatric patient in an acute care setting to proving the value of case management in a changing healthcare environment.
